Suggestions:

Title - spelling of convention
'about each item listed' (period; maybe just drop 'for the convention')
'If you didn't turn in at the convention...' ('If you didn't turn your
evaluation form in at the convention registration desk, please mail it
to:')
'will help to make next year's convention' (apostrophe)
'Saturday entertainment' and 'Entertainment Saturday' (twice)
'bylaw' (should be plural)
'Green Hours volunteer' ('Green Hours volunteering' or 'Concept of Green Hours')
Capitalize consistently (if Convention is/isn't capitalized, then all
throughout, etc.)

Maybe 'How did you find out about the GRP Convention?'

Comments:

Was there really much choice regarding the date for the convention this
year vis a vis national (nominating) convention?

As an aside, I didn't pursue any of the speakers I approached before about
next year since we don't have a date set (and it'll take a while before we
do, probably).

Think all the important bases are covered. People can write whatever they
like in the comments area.
